Liability Waiver for All Classes

As a willing participant in Music Together® classes at Little Music Makers, my family and I agree to the following:

ASSUMPTION OF THE RISKS: I understand that naturally occurring disease processes (including, but not limited to, the novel coronavirus or COVID-19), may exist in the community, including at Little Music Makers, and present an inherent risk that cannot be eliminated. I acknowledge that, although Little Music Makers is taking reasonable measures to avoid contact, transmittal, and exposure of disease processes like COVID-19 between people in our classes (including Music Together^® teachers, and third parties), it is ultimately my own responsibility to ensure that I take appropriate actions to safeguard myself and my child/children. I understand and agree that as a registered family with Little Music Makers, I am accepting and voluntarily assuming the risk that my child or I may be exposed and become ill as a result of COVID-19 and other diseases. I also assume the inherent risk of being outdoors with my children including but not limited to ticks, bees/yellowjackets, insects, poison ivy, or other outdoor hazards.

WAIVER OF LIABILITY: I, on behalf of myself and my children, hereby voluntarily release, forever discharge, and covenant not to sue LIttle Music Makers and its owners, managers, contractors, members, employees, agents, and volunteers, or the location owner (“Released Parties”) for any claims that may arise out of or relate in any way to my or my child’s exposure to any communicable disease, including but not limited to the novel coronavirus (COVID-19) or any other accident that may cause harm. I agree not to hold Little Music Makers, Michelle Meyers, and its representatives responsible for injuries or accidents in connection with Little Music Makers practices or activities on or off-site. If any injury appears serious enough for hospitalization or medical attention, such expense will be borne by the parents or guardian. I agree that Little Music Makers is not responsible for personal items (including cell phones) broken or left at Little Music Makers teaching facility or activities (including but not limited to classes, playdates, etc.).

The claims hereby released include, but are not limited to, claims of negligence against any of the Released Parties. I understand and voluntarily assume all risks inherent in the nature of this activity and I waive and release all claims, costs, liabilities, expenses and judgements against Little Music Makers and its representatives arising out of my participation or my child’s participation in Little Music Makers activities. 

RIGHT OF REFUSAL: Failure to follow policies posted online or communicated via emails, text messages, class instruction, etc. may result in expulsion from the program/classes/center at the Director's discretion with no refund. Little Music Makers reserves the right to refuse service to any individual or family not following the policies.

I HAVE CAREFULLY READ THIS WAIVER / RIGHT OF REFUSAL AND FULLY UNDERSTAND ITS CONTENTS. BY ENROLLING IN CLASSES AT LITTLE MUSIC MAKERS, I UNDERSTAND I HAVE GIVEN UP CERTAIN LEGAL RIGHTS AND THAT THIS IS A BINDING LEGAL DOCUMENT
